STAGEWISE MEDICAL TREATMENT OF MALE PATTERN BALDNESS

In Today’s post, we will help you locate your stage of baldness on the international Norwood’s scale and suggest medical hair loss treatments for every stage for reversing baldness stage-wise backwards!

Will will not go into the science and mechanism of male pattern baldness in this video. We have discussed all these in our previous episodes.

Let’s get started!

Baldness is staged commonly using the International Norwood’s scale, the hair loss stages are described with a number ranging from 1 to 7 depending on the severity of hair loss.

Let’s start with:

Norwood Stage 1: This type describes the least amount of hair loss with an adolescent type of hair line. You can consider this normal unless you have a family history of male pattern baldness.  If you have a family history closely monitor your hairline to determine if your hair loss develops into stage 2.  And No Treatment is necessary at for stage 1.

Norwood Stage 2: This stage means a small amount of hair loss with progression to the adult type of hair line.  Here there is slight recession of hair line around the sides or temples.
Regarding Treatment for this stage. If you have family history, first consult a doctor to verify that your hair loss is caused by male pattern baldness. There is a good chance that, with treatment, you can regain most of your lost hair and prevent progression into stage 3. You can start with simple treatment like 2 percent topical minoxidil application at this stage or start rolling a 0.5mm derma roller once a week with or without minoxidil solution. Norwood Stage 3: This stage describes a mild to moderate amount of hair loss. This is the earliest stage of hair loss considered cosmetically significant enough to be called “baldness” according to Norwood scale. At this stage, most men show a deep bilaterally symmetrical recession of hairline at the temples that is sides, which are either completely void of hair or only sparsely covered with hair.  This resembles letter M, U, or V shape. The vertex figure here shows the additional thinning of the hair at the crown of the head.
This is the most important stage of norwood, because you must start treatment as early as possible to prevent progression into stage 3. Remember, The higher the stage, the more difficult the treatment is.

Well, the treatment options at this stage, you may require a combination therapy like minoxidil 5 percent twice daily plus finasteride 1 mg tablet. This stage is stubborn for most people. If you are able to control or grow back hair at this stage, the baldness progression is generally slowed down in later part of your life.
Also the other treatment options you can consider at this stage are PRP and mesotherapy along with minoxidil. You can watch my detailed post on this PRP mesotherapy.

You can also consider Hair transplantation treatment at any stage from stage 3 onwards.

Norwood Stage 4: This stage describes a moderate amount of hair loss. At this stage there is further deepening and recession at the front of the head as well as the temple areas. Hair loss at the crown is also prominent now and a thick bridge of hair will separate hair loss at the front of the scalp from that at the vertex or the crown of the head.  To differentiate this from stage 3, you can draw an imaginary vertical line tangential to the front margin of the ear. If hair loss crosses behind this line, then is stage 4.
Regarding the treatment options at this stage, there is no guarantee that using all the treatments including finasteride, minoxidil or a combination therapy will bring back all your lost hair. But the good news is if you start early treatment, the miniaturized hair will come back to normal and you can switch back to stage 3. You might have to use minoxidil in a higher concentration like 10 percent plus finasteride 1mg daily indefinitely.  Please check my article on DHT to understand this miniaturization and the mechanism of hair loss.

You can also treat this stage with PRP and mesotherapy plus Stem cell therapy and consider hair transplant option at this stage too.

 

Norwood Stage 5: this stage marks the beginning of severe degree hair loss. In this stage, there remains only a thin separation between the loss of hair at the hairline and the loss of hair at the crown, the band of hair between the two is much thinner and narrower. Hair loss at both the crown and the temporal regions are larger and more distinct.   The treatment options at this stage are same as that in stage 4.

 

 

Norwood Stage 6: In this stage there is a large amount of hair loss. The bridge of hair that once separated the front of the head from the crown is now almost fully lost, only a few sparse strands may remain. The remaining hair now forms a horseshoe shape around the baldness concentrated in the center of the scalp. Hair loss on the sides of the head will also extend further at this stage.
At this level, a hair transplant may be the only option.

 

Norwood Stage 7:  The most advanced stage of hair loss, Here, only a wreath of thin hair remains on the sides and back of the scalp. This stage has the least chance of regaining your hair with any treatment.

The company RepliCel was first set up in Japan in 2013 and now in partnership with Shiseido, together have brought a  lot of changes since then.  Shiseido, has currently funded a human clinical study of RCH-01 at Tokyo Medical University Hospital and Toho University Ohasi Medical Center in Japan.

With the clinical study of RCH-01 now successfully completed in Japan, RepliCel expects Shiseido to declare its near-term plans for RCH-01.

RCH 01 has Alternative Names: DSC cell therapy – RepliCel; Hair cell replacement therapy – RepliCel; Human autologous hair follicle cells RepliCel Hair-01

This RCH-01 is based on the principle of advanced cellular regeneration involving the dermal sheath cup cells, hence its also called DSC cell therapy.

Replicel’s RCH-01 hair growth treatment is done by first extracting and then culturing a person’s own hair follicle cells and then re-injecting the modified DHT resistant hair follicle cells back into their scalp.

What they do is Firstly, through a simple punch biopsy, they isolate the cells from healthy hair follicles from the patient’s scalp. Then the hair follicles are dissected and the dermal papilla portion is cut off. This dermal papilla is the region where the DHT receptors are located and are responsible for damage by the DHT – that’s dihydroxytestosterone.  If you have n’t watched my DHT and hair loss episode, please check that from an endscreen link to understand this mechanism.
Well, These cells are removed from the remaining follicle and cultured in a growth medium. The cells are replicated or multiplied into millions of new cells and then these are drawn into a syringe. This syringe is inserted into the patients scalp skin and injected back into the patient.  Remember, these are patient’s own replicated cells and hence no adverse reaction or side effects and this is very much similar to PRP treatment for hair loss, where patients own platelet rich plasma is injected. Well, These cells after injection into the scalp, scatter and migrate to attach to the dormant hair follicles and rejuvenate and stimulate them to grow further and also they have the ability to induce the formation of new healthy hair follicles and multiply over a period of time to give rise to new hair which emerges out of the skin surface.

More details should be disclosed by the company once they launch the product.

The Current Status of the product: Shiseido’s Japan phase 1 clinical trial started in 3rd quarter 2016 is now complete as per company’s website and news. Now whether they enter into phase 2 or directly launch the product. Its still not clear. But likely possibility is the treatment could become available in 2020. Because of the strong positive data from this trial, there is a strong possibility that Shiseido might decide to launch the product in Japan first. So keep your fingers crossed for the launch.

Stage wise Hair loss Treatment in Women – Female Pattern Baldness

In Today’s article, we will look into the stages of female pattern baldness and the best treatment options for hair loss in women. In my previous article, we discussed on the causes and mechanism of female hair loss and hair thinning and the telogen effluvium.

The pattern of hair loss in women is different from that in men. We have discussed male pattern baldness and its stage wise treatment in earlier episodes. Now we will discuss stages wise treatment of female pattern baldness – that’s again androgenetic alopecia.

The male pattern hair loss is typically seen at the front of the hairline and at the crown of the head, whereas women with androgenetic hairloss more often have diffuse thinning on all areas of the scalp but thinning starts typically at the front portion of the scalp.  Remember the reduction in hair thickness and density leads to a reduction in the overall hair volume.  But complete baldness in women is very rare.

The stages of hair loss in women is measured by different scales. One simple and the conventional scale used is the Ludwig’s scale. This scale has three stages. In all three stages, the hair line is preserved unlike the male pattern baldness, where hair line recedes and then completely lost.

Stage – 1: There is minimal thinning of hair at the front portion of the scalp as you can see in the illustration. This type of hair loss can be easily covered or hidden by hair styling techniques. But this does not mean that you should not start treatment at this stage.  We will discuss treatment options later.

Stage – 2: There is moderate thinning and noticeable widening of the mid-line part.

Stage – 3: There is much more diffuse thinning, with a see-through appearance on the top of the scalp or the thinning is seen over the entire scalp.

Now lets discuss on the treatment options for hair loss in women.

Firstly you need to confirm this is androgenetic alopecia and rule out other causes of hair loss like PCOS, thyroid disease and other causes as discussed in my previous article.

The first line of treatment for female pattern baldness is:

  1. MINOXIDIL solution: This is the first and foremost medication used for the treatment of hair loss in females as well as males and easily available over the counter medication without any prescription. This is an FDA approved medication for hairloss. In women generally 2 % or 5 % minoxidil applied on the affected area of scalp once or twice daily is used. 2 % is used for the treatment of Stage 1 and 5% solution is used for stage 2 and 3 . This is important to prevent you from going into stage 2. The higher the stage, the more difficult the treatment is. And remember minoxidil is not a miracle drug. You need to consistently apply it for alteast 3 to 6 months to see some obvious improvement. If minoxidil works for you, then you need to keep using it daily to maintain those good results. If you stop abruptly, you’ll start to lose hair again. Also You must know how to properly use minoxidil solution. Use the dropper or the spray pump and apply on a dry scalp on the affected area and gently massage with your fingers in a circular movement. Do not over do it and also prevent it from going into your eyes. Then wash your hands thoroughly after use. Do not shampoo you hair for at least four to six hours after application. If this is causing irritation on you scalp, then its more likely a contact dermatitis. You might need to consult your doctor. It actually not caused by minoxidil, but the alcohol content in minoxidil solution causes this irritation.
  1. Anti Androgens: This type of treatment has to be taken by consulting your doctor. Some women who don’t respond to minoxidil are prescribed with the anti-androgen drug spironolactone for treatment of androgenic alopecia. This is usually prescribed to women with polycystic ovary syndrome (PCOS) because they tend to make excessive androgens that cause hairloss as discussed in my previous episode.

 

  1. Hair transplantation: This is the final option, if no treatment works.

Male Pattern Baldness – Early Signs, Stages and Treatments

In Today’s article on hair loss, we will look into some important early warning signs to pick up male pattern baldness and start early treatment to prevent further hair loss or even reverse hair loss as early as possible. We will also help you locate your stage of hair loss on the commonly used international Norwoods scale and discuss on what hair loss treatment to use for that particular stage.

Lets begin!

Picking some early signs can certainly help you combat male pattern baldness either to stop further progression of hair loss or even reversing hair loss. And remember – Prevention is always better than Cure!  First of all, try to assess yourself whether you are prone to male pattern baldness or not by considering these important factors:

  1. Age: Male pattern baldness or androgenetic alopecia can start at any age after puberty around the age of 15. Roughly, your risk to baldness is 20% in your twenties, 30% in your thirties, 40% in your forties ties and so on, that’s rising 10% for every decade of your life. Not only your age, but you have to watch out other factors too to deal with baldness prevention.
  2. Genetics: If you have a strong family history, you have to be double or triple cautious to apply preventive steps. And Hair loss, as discussed in my previous episodes – its polygenetic – meaning multiple genes are involved, from maternal side or paternal side or a combination. You might have to even look at your uncles from both sides to assess your risk.
  3. Other Factors like Stress, Smoking and Nutritional deficiencies are also important in adding that risk for baldness. Now! Lets look into the early warning signs of balding. We all know that balding can be a traumatic process for everyone. The one who is pretending to be cool with his baldness is a big liar. Well, the good news is by spotting the early warning signs of balding, you can start applying some preventative measures to secure your hair. Yes nothing is impossible!

So, how do I know whether I am going bald or will become bald? We use the International scale for staging of baldness in men – that’s called the Hamilton-Norwood Scale for androgenetic alopecia or the male pattern baldness. Switching back to an earlier stage with Treatment:

Yes this is possible with the treatments at every stage as discussed. Though these treatments will not restore a full head of hair, but these can certainly help one to climb back into a lesser stage of the Norwood scale. For example, with stage 6 hair loss, a reasonable outcome with treatment is a return back to stage 5 or even 4 if proper treatment is started.

10 Bad Habits That Cause Hair loss and Hair Thinning

Certain everyday habits can lead to hair thinning and cause hair loss. If you do not stop them right away, you can become bald in no time.

 

10: Hot Showers: Hot water dehydrates hair strands making them dry and brittle by stripping away your hair’s natural oil coatings. These hair are prone to snap and fall out.

05: Diet: Lack of Balanced Nutritious food. Healthy lustrous hair require you to consume diet full of vitamins, minerals and proteins.

04: Excessive Scratching your Scalp during Itch – can also lead to hair follicle damage.

03: Vigorous combing can cause traction alopecia as discussed in point number 4.

02: Excessive Use of hairstyling products: Many styling products can cause more harm that good. This is due to high alcohol content in them, they can make hair dry and brittle.

01: Medications: Yes if you are on certain drugs, this can cause hairloss.  Drugs like statins, anti-depressants, anti-anxiety agents, anti-hypertensive medications  or hormones (like thyroid replacement drugs) can cause hair loss, as well as birth control pills, can disrupt or interfere with the normal cycle of hair growth, causing hair to go into a resting phase and fall out prematurely.

ONION JUICE FOR HAIR GROWTH TREATMENT – Does it Work?

Onion Juice for Hair loss treatment? It might sound surprising, but onion juice is considered a cheap home remedy for hair loss. In todays episode, am gonna reveal you the real facts on onion juice as a remedy for hairloss. The most important question whether it works for male pattern baldness, that’s androgenetic alopecia which is the most common cause of baldness and hair loss – over 90 percent of the cases.

Before I reveal you about the efficacy of onion juice for baldness and on which type of hair loss it really works and how? Lets quickly learn some different types of alopecia. Alopecia is a medical term for hair loss. Do not skip this topic, as this is important to understand the role of onion juice for hair fall treatment.

  1. Androgenic Alopecia – or hair loss caused by genetics, or family history. This is the most common type of hair loss pattern in men and also called male pattern baldness in males and again the most common cause of hair loss in women and is called female pattern baldness. We will discuss these types and stages of male pattern baldness in detail in some other episode.
  2. Alopecia Areata – This is a patchy hair loss and is an autoimmune disorder which causes the bodys own immune system to attack the healthy hair follicles. The exact cause of this remains unknown, but it is thought to be triggered by stress and traumatic events.
  3. Alopecia Totalis and Alopecia Universalis are rare types: Totalis means the patchy type of hair loss will make the scalp totally bald. Where as the Universalis means loss of all hair from the entire body including eyebrows, eyelashes and all body hair.
  4. Traction Alopecia: This type of hair loss occurs due to traction or excessive tension by pulling of the hair like when vigorously combing hair, applying some hair styling procedures and even tight braiding and tight ponytails.  This type of hair loss is usually reversible when you stop these bad habits that cause hair loss. Now coming straight to the point! Now you know the types of hair loss, we will now know the real facts and myths on onion juice for baldness treatment.

Firstly Some Facts on Onions:

  1. Onions and even garlic contain sulfur, which is good for hair because the hair protein keratin need sulfur for its production. It is also said that the sulfur from onions may also help promote collagen production. Collagen in turn helps the production of healthy skin cells and hair growth.
    Here is the catch! You need dietary sulfur intake which is then carried through blood circulation into the hair bulb which then helps in promoting hair growth. So topical application of onion juice has absolutely no scientific basis.
  2. Other theories say that it has anti microbial and anti oxidant properties that help in hair growth.
  3. Some also say it increases blood circulation and promotes hair growth, but nothing is scientifically proved.
  4. And moreover, only one research study was conducted in 2002 on this onion juice and treatment of alopecia areata, mind you it was used only for patchy hair loss and not for the commonest androgenetic alopecia. In alopecia areata it was shown to give some successful results. Although the study said it showed positive results, it was tested in a small number of  only 38 participants.  I have linked two research articles on this topic and both say – the exact mechanism of action of onion juice for hair loss treatment is not known. So that says it all!

And No other scientific study has shown the effectiveness of onion juice for male pattern baldness or hair loss.  If you have patchy hair loss or even traction alopecia, it might work, whether due to a placebo effect or by other means, not sure!

    Well, the next precaution is:
  2. Never use somebody else’s derma roller on yourself. This can transfer infections including HIV, hepatitis B and hepatitis C, though its very rare if safety precautions followed properly.

NEW DRUG WAY-316606 Grows Hair in 2 Days | Latest Health News 2019

Hello Friends and Patients. Today am extremely excited to present this latest innovation in dermatology – a new drug developed by scientists that can cure baldness, treat hairloss and grow hair by follicular stimulation as early as in two days.

Hair loss and baldness is a common disorder and can lead to psychological distress.  Till now, there are only two proven drugs that help promote hair growth but too gradually. These are Finasteride 1mg only for men and Minoxidil solution 2 % to 15% for both men and women. Finasteride is known to produce erectile dysfunction and loss of libido as its side effect and minoxidil can cause reversal hair loss if you discontinue its use.

Recently a paper authored by a scientist from the Department of Dermatology at the University of Manchester have presented with a new compound, called WAY-316606 which was originally developed for osteoporosis. This drug enhanced hair growth in samples of human hair follicles from hair transplant patients in a small study and the effect was seen as early as in just two days.

The team discovered its effectiveness by first testing a drug called cyclosporin A, that’s been shown to boost hair growth in the past. The problem with cyclosporin A is its an immunosuppressant drug that’s usually used on people receiving organ transplants and cancer patients. So,  it’s too toxic for regular use.  During this process, the team analyzed how it promotes hair growth on a molecular level and then discovered that WAY-316606 can do the same thing but without side effects.

So the Good news is the drug WAY-316606 may soon be entering human hair growth clinical trials with the help of an Italian pharmaceutical company.

The Bad news is: The concern with the new drug, WAY-316606. And that it affects the regulation of the body’s Wnt pathway, in which aberrant regulation is known to be linked to increased cancer risk. But the scientists say they still do not know if this particular drug will increase cancer risk.

In the paper, the researchers acknowledge the dangers of messing with Wnt, but they explain that the drug’s specific mechanism of action inside cells might allow it to avoid cancer.

So, all baldies, keep your fingers crossed. If this drug passes the trials and approved for use on humans, this will revolutionize the treatment of hairloss and cure baldness in days.
